Guidelines for Submitting the RPSU Student Request Form

Please read the following guidelines carefully before submitting your request.

1. Enter your Student ID, name, university email, and program correctly.
2. Select the appropriate request type.
3. Upload a clear and complete Student ID card.
4. Provide your signature before submitting.
5. Submit accurate information and genuine requests only.
6. Do not submit duplicate requests for the same issue.
7. Check all information and attachments before clicking Submit.
8. For Change Password your personal email must first be updated in the ERP. If your contact details are outdated, request Change of Contact Details first. After the update, submit the Change Password request. The new password will be sent to your updated personal email.
9. For urgent matters, visit the concerned department/office directly.
